Part 3 The Patient in Silent Hill Townfall is the chapter of the game where things start to get seriously messed up, more than they already were in Part 2. This part of the game also acts as a quick class in what it takes to be a doctor, as you'll learn to perform x-rays and administer medication appropriately.

It's quite fun if you're able to ignore the danger that lurks in the hospital's halls… To help you out with this chapter of the game, here's my Silent Hill Townfall Part 3 The Patient walkthrough.

Need more help navigating St. Amelia? Here's our Silent Hill Townfall tips, tricks, and guides.

Silent Hill Townfall Part 3 The Patient walkthrough

Inside of St. Amelia Medical Centre now, you'll be temporarily playing as Zoe again until later stepping back into Simon's shoes. I also hope you're not squeamish, as this part of Townfall sees you handling lots of medical equipment, medication, and even handling blood transfusions. Fun.

Below, you'll find all main objectives during the Part 3 of Silent Hill Townfall, as well as exactly how to complete them.

  1. Answer Dr Glenn's questions about the patient.
  2. Perform an x-ray on the patient.
  3. Find the three medications for the patient.
  4. Administer the medication to the patient.
  5. Collect the intubation equipment, and intubate the patient.
  6. Explore the basement, and collect Alan Duncan's blood bag.
  7. Collect the blood bags from Ward SW.
  8. Collect the blood bags from Ward DP.
  9. Place the blood bags with the correct medical records.
  10. Solve the blood transfusion vault puzzle.
  11. Start again.

1. Answer Dr Glenn's questions about the patient.

You will now be playing as Zoe in the Surgery of the Medical Centre, and shortly, Dr Glenn's voice will begin booming through the intercom, requesting Zoe talk to him.

Before you do that, loot the room. You can grab the following:

  • There's a First Aid Kit by the sink.
  • The St. Amelia Medical Centre Map is on the wall beside the whiteboard.
  • The PC contains information regarding equipment, x-ray dosages, and medication dosages that we'll need later on.
The Medical Centre map is shown in Silent Hill Townfall.

Now, speak to Dr Glenn. He'll ask you to answer a few questions about the patient and you'll need to read the medical equipment around the room correctly to do so. Unless you've a background in medicine, or happen to have been on the receiving end of a lot of medical care, these surgery answers aren't the easiest to discern without prior medical knowledge.

His questions, and how you should answer, are as follows:

  1. Dr Glenn asks for the patient's blood pressure, but there are five blood pressure monitors in the room. You want to go for the 'most sensible' sounding option and read the side of the monitor that says dystolic/systolic on it. The correct monitor is the second one from the right (this is on the right-hand side of the bed, if you're facing the patient).
  2. Dr Glenn asks for the patient's pulse next. The correct monitor is the third one from the right, which shows a pulse of 162.
  3. Dr Glenn now asks for the patient's blood oxygen levels. The correct monitor is the fifth one from the right, on the left-hand side of the bed, which shows a blood oxygen level of 80.
A vitals monitor is shown in Silent Hill Townfall.

After answering the three questions above, Dr Glenn will then prompt you to perform an x-ray on the patient. He will Pager you the code to the x-ray room that's connected to the surgery: it's 7103.

2. Perform an x-ray on the patient.

Enter the x-ray room using the 7103 code, at which point Dr Glenn will tell you to x-ray the patient's head. However, the x-ray machine itself has a lot of buttons, and you don't want to input any old option.

Zoe is shown interacting with an X-Ray machine in Silent Hill Townfall.

Instead, it's now time to refer to the X-Ray Dosage Table on the Surgery's PC. This tells you exactly what Volts (kV), Cur (mA), and Time (s) to set depending on which body part you're scanning.

The X-Ray Dosage Table is shown on a PC in Silent Hill Townfall.

As we're scanning the patient's skull, the options you want to select are as follows:

  • Volts (kV) = 3
  • Cur (mA) = 7
  • Time (s) = 1

Once the x-ray settings have been established, use the computer to align the camera with the patient's skull.

Zoe is shown aligning the X-Ray camera with the patient's skull in Silent Hill Townfall.

After the camera has been aligned, press the 'Alignment' button on the x-ray machine.

Up next, you'll be tasked with scanning the patient's chest. The dosage values you need for this x-ray are as follows:

  • Volts (kV) = 7
  • Cur (mA) = 3
  • Time (s) = 1.2

Once again, align the x-ray camera with the patient's chest and then press the 'Alignment' button to conduct the scan.

Afterwards, collect both x-ray images from the Printer in the room. You can then place these on the whiteboard beside the intercom in the Surgery.

Zoe is shown looking at some x-rays in Silent Hill Townfall.

Tell Dr Glenn about each x-ray image when prompted, and he'll then task you with putting together and administering a cocktail of medications for the patient. That said, none of the medicines are in this room, and we'll now need to scour the rest of the hospital for them.

3. Find the three medications for the patient.

The medications you need to find are as follows (but they're fortunately written down on your map too):

  • 2.5mg Dextracyclate
  • 3mg Hydroxalene
  • 11mg Tensobromide

Now, to make things as simple as possible, I'm going to list exactly which rooms to go to that contain these medicines below. I'll also detail some other rooms of note that you ought to visit too, but I won't exhaust every room in the hospital here. By all means spend some time exploring the rest of the hospital though, as there's various healing items to grab and hiding spots to jump into if needed.

  1. The Pharmacy Store on the ground level (east of the Surgery), contains Tensobride 11mg.
  2. The Reception and the Office (in the hallway outside of the Surgery) each contain a save phone.
  3. Upstairs in the Chapman Ward (in the west of the hospital), Bay 6 contains Dextracyclate 2.5mg.
  4. Also upstairs in the Chapman Ward, the northern Office contains the Records Room code: 2605.
  5. Go downstairs and enter the ground level of the Chapman Ward. Use the Records Room code to enter the Records Room and collect 3mg Hydroxalene from the storage locker.
The hospital's Records Room code is shown in Silent Hill Townfall.

After collecting all three medications, you can return to the Surgery and begin administering these to the patient.

4. Administer the medication to the patient.

You'll now need to use the infusion equipment around the room (shown below) to administer medication to the patient, but it's not as simple as attaching the medication to an IV. Before you can give the patient their meds, you need to set the right delivery rate on the infusion equipment depending on the patient's Body Mass.

For this, you'll need to refer to the Medication Dosage Table on the PC, and the patient's Body Mass on the whiteboard, which is 88kg.

Zoe is shown using the infusion equipment in the Surgery in Silent Hill Townfall.
The Medication Dosage Table is shown on a PC in Silent Hill Townfall.

Using the Medication Dosage Table and the patient's weight of 88kg, the associated delivery rates for each medication/his BM are:

  • 2.5mg Dextracyclate = 15.
  • 3mg Hydroxalene = 7.
  • 11mg Tensobromide = 8.

On the machines, press 'set rate', input the above delivery rates depending on the medication being administered, then press 'ready' and move onto the next medication.

When all three medications are ready, interact with the pump machine to the left of the patient's bed and press 'start'.

Up next, the formidable Dr Glenn says you'll need to get equipment from the basement to intubate the patient's breathing, but will need to call ahead and let them know you're headed down there.

5. Collect the intubation equipment, and intubate the patient.

Go to Reception next and call the extension shared with you by Dr Glenn: 5353. The person on the end of the line is less than happy to be hearing from you, but says we can find the intubation equipment that we need down in the basement.

Enter the elevator outside of Reception next and press 'B'. You'll now need to carefully make your way to Storage on your map, where you'll find a Medical Intubation Kit on one of the shelves.

The Medical Centre Basement map is shown in Silent Hill Townfall.

Return to the Surgery via the elevator next and interact with the patient to begin intubating them. What follows is a minigame in which you need to position the tube in the correct spot, and you'll know you're in the correct spot if the indicator in the lower right-hand corner of your screen begins to shake and glow white.

Zoe is shown intubating a patient in Silent Hill Townfall.

On top of that, arrows will appear showing you where to position the tube after a few failed attempts, so don't fret too much if you can't get this one quite right straight away.

After intubating the patient, Dr Glenn now wants you to perform a median sternotomy which involves making an incision along the chest. A cutscene follows in which the patient is revealed to be Simon.

6. Explore the basement, and collect Alan Duncan's blood bag.

After the cutscene, you'll find yourself playing as Simon in the Surgery now. First, enter the x-ray room using Zoe's code from earlier (7103) and collect Strange Postcard 7 from the desk in there.

Next, go down the ladder that's appeared in the Surgery room.

You'll find yourself in a room with a door in front of you that has a tally count on it again. Go through the door and along the hallway ahead. The door at the end of the hall is locked, so head right, and you'll soon come to a gap in the wall beside some wooden pallets that you can squeeze through.

Simon is shown approaching a gap he can squeeze through in Silent Hill Townfall.

At the end of the hall, use your CRTV to tune into the Radio Signal in this room. You'll hear a nurse explaining that they can't help any more patients, and the Medical Centre is shown.

When you're ready to move on, exit via the vent in this room to reach a bathroom.

Exit the bathroom and go left. Here, you can swap your Plank for the Pipe that's barring the gate shut. Go through the door opposite next, and along another hallway.

In the next room, you'll be able to collect the Medical Centre - Different Map, and there's a save phone along the wall for you to make use of.

The 'different' map of the Medical Centre is shown in Silent Hill Townfall.

If you explore the room, you'll find multiple blood transfusion machines alongside either blood bags or medical records. It's now time to kick off the blood bags puzzle, which tasks us with finding different blood bags in these strange wards and putting them beside the corresponding medical record.

First things first, explore the different rooms here as you please. There's some healing items to grab but otherwise, you want to enter the rightmost room on the south side of the eastern hallway. Inside this room, there's a vent to travel through that takes you to the leftmost room on the south side of this same hallway.

Collect Alan Duncan's Blood Bag from beside the bed here, and read Zoe's Patient List on the table. This reveals that there are four more patients – two in each wing – that we need to find in this area. They are as follows, and will be marked on your map after interacting with this note:

  • Sarah Watson - Room S05, Ward S.
  • Lesley Greer - Room S12, Ward W.
  • Matthew Clark - Room D01, Ward P.
  • George McEwan - Room P10, Ward D.
Zoe's Patient List is shown in Silent Hill Townfall.

You'll also find a recorded message from Zoe on the table in which she discusses her patient, Alan Duncan.

7. Collect the blood bags from Ward SW.

If you look at your map, you'll see two wards either side of the room containing the Blood Transfusion Machine. First, we want to go through the western gate leading to Ward SW. To do this, place Alan Duncan's Blood Bag beside the shuttered gate leading to the ward to open it.

Simon is shown looking at a blood transfusion machine in Silent Hill Townfall.

Go down the ladder in front of you, and if you go to Lesley Greer's room on your right, you'll find that the door is being blocked from the other side.

Enter Room 11 next and go through the vent opposite the door to reach Room 10.

From here, head south and into Room 05, which is Sarah Watson's room. Collect Sarah Watson's Blood Bag from beside the bed, and listen to Zoe's voice message on the desk. Now, it's time for the scary part of this section…

Go down the stairwell in the west of Ward S to reach Ward W. The huge bulbous enemy that chased Simon into the Medical Centre is present now, and you'll need to stealth past them and into Room 04. Here, you'll find the Ward W Master Key.

Simon is shown looking at an enemy on his CRTV in Silent Hill Townfall.
The Ward W Master Key is shown in Silent Hill Townfall.

With the Ward W Master Key in your possession, beeline for Room 13 next. Go up the ladder here to finally reach the interior of Lesley Greer's room, and collect Lesley Greer's Blood Bag from beside the bed.

You can now open the vent in Room 13 to access Room 11 again, then make your way back to the main hall. Of course, feel free to explore every room and loot away if you wish before exiting. For what it's worth, Strange Postcard 8 is located in Room 06 in Ward W.

Now it's time to collect two more blood bags from Wards D and P.

8. Collect the blood bags from Ward DP.

To access Ward DP, you need to use another blood bag on the transfusion machine beside the eastern shuttered gate in the main hall, but you can't use just any blood bag. As mentioned earlier, we need three blood bags for the blood transfusion puzzle in the main hall, and these blood bags need to match the medical record it sits beside.

As a reminder, the two blood bags we currently have and the clues attached to them are as follows:

  • Lesley Greer. Blood Type B-. Flashes of light/hand waves. Nothing moves.
  • Sarah Watson. Blood Type A+. Rhythm holds. Head doesn't.
Lesley Greer's blood bag is shown in Silent Hill Townfall.
Sarah Watson's blood bag is shown in Silent Hill Townfall.

As for the medical records in the main hall, one of them refers to a female with a head wound, and it's safe to assume this is Sarah Watson.

With that in mind, use Lesley Greer's blood bag on the transfusion machine and head into Ward DP.

In the Ward, look at your map and go into Room 03. There's a path at the back of the room that takes you into Matthew Clark's room, Room 02, where his Blood Bag is in the elevator. Interact with this and the Blood Bag will be dropped into Room 03 of the below floor, which is Ward P.

Simon is shown interacting with an elevator in Silent Hill Townfall.

Next, go to the stairwell on the east of the ward. Just south of it is a vent you can enter allowing access into Room 06 and Room 07. In Room 07, you can jump down into George McEwan's room, which is Room 10 in Ward P.

Simon is shown preparing to jump into a room below him in Silent Hill Townfall.

Here, you'll be able to collect George McEwan's Blood Bag, as well as the Ward P Master Key. Exit the room now, and use the key to enter Room 03 on this floor, where you can finally collect Matthew Clark's Blood Bag from the elevator.

After collecting the final blood bag, all gates in the area will be closed, making finding our way back to the main hall a little trickier. On top of that, the bulbous enemy is lurking here now, so you'll need to avoid them while making your escape.

Make your way into Room 05 in Ward D (upstairs), which is now accessible, and turn the valve there. (You can reach this room by going into the vent in Room 02 of Ward P, then up the stairs.) If you're interrupted by the monster, hop into the cupboards in this room to hide. Then, once the coast is clear, finish turning the valve.

Simon is shown turning a red valve in Silent Hill Townfall.

After this, you'll then need to enter Room 10 on the opposite side of this floor to turn a second valve. Again, hide after turning it, as the nearby monster will come to investigate.

Turning these two valves finally opens the gates again, allowing you to beeline for the ladder that leads back to the main hall. Just be careful of the monster as you do that…

Just like earlier, feel free to explore the remaining rooms using the Master Key before leaving if you're brave enough. You'll find Strange Postcard 9 inside Room 08 in Ward P.

9. Place the blood bags with the correct medical records.

Once back in the main hall, it's a wise idea to save your game. Then, it's time to finally solve the blood bag puzzle, which you can do by placing the three blood bags in your possession beside the correct medical records.

Here are the three blood bags we currently have, and the clues written on them:

  • Sarah Watson. Blood Type A+. Rhythm holds. Head doesn't.
  • Matthew Clark. Blood type B+. Words scatter. Body complies.
  • George McEwan. Blood type AB+. All over. Nothing left to cool.

And here's where to place all three blood bags:

  • Put Sarah's Blood Bag beside the medical record that says 'Regular Pulse and Head Wound'.
  • Put Matthew's Blood Bag beside the medical record that says 'Obeys commands'.
  • Put George's Blood Bag beside the medical record that mentions '2nd/3rd' degree burns.

After placing the blood bags, interact with the Blood Transfusion Machine to 'transfuse' the blood bags, and the door beside the machine will open.

10. Solve the blood transfusion vault puzzle.

Just through the newly opened door is the blood transfusion vault puzzle with Simon Ordell's name in the centre. His name is surrounded by various initials, and you can interact with each ring of the vault to create paths between the initials and Ordell's name. On top of that, the walls of this tiny room are covered in plaques that tell you different blood type combinations.

The blood transfusion vault puzzle is shown in Silent Hill Townfall.

The goal of this puzzle is to create Simon's blood type using a combination of other blood types, which are directly tied to the patient's blood bags in the main hall. Fortunately, Simon Ordell's blood type can be seen on his medical wristband in his inventory: he has a blood type of A+.

If you then look at the plaques on the wall, you'll find that to get a blood type of A+, you must combine the following blood types with Simon's blood:

  • O-
  • O+
  • A-
A plaque showing which blood types can be combined to create A+ is shown in Silent Hill Townfall.

A+ is also shown on the plaque, but Simon will provide that, so you only need to worry yourself with hooking up patients who have the above blood types to Simon's name in the middle of the vault.

You can check each patient's initials and their blood type by going back and referring to the blood bags or medical records in the main hall. I've noted their blood types here too to help:

  • HM = O-
  • SW = A+
  • RD = AB-
  • MC = B+
  • GM = AB+
  • TF = O+
  • SH = B-
  • DH = AB+

With the above in mind, we need to hook the following peoples blood up to Ordell via the vault puzzle:

  • HM
  • TF
  • SW

To hook these initials up to Ordell in the middle of the vault, you must rotate each part of the dial so that the blood of the aforementioned initials can reach the center (and nobody else's blood!). See the below image to see how I arranged the vault, and make sure to press Ordell's name in the middle once you're done.

The blood transfusion vault puzzle is shown in Silent Hill Townfall.

After solving the puzzle, the door is opened and you've yet another spooky hallway to go down. At the end, open the door using the Room 204 Key. A cutscene will follow.

11. Start again.

After the cutscene, Simon will have found himself waking up on the shorefront all over again. Fortunately, you can at least run this time unlike earlier. Though, the downside is that the water is full of monster patient corpses now…

The shore of St Amelia is shown covered in corpses in Silent Hill Townfall.

Go along the dock just as you did at the very beginning of the game and approach the sign for St. Amelia, which is now blank.

That marks the end of Part 3 The Patient! For more Silent Hill Townfall, take a look at my Part 4 Clara Woods walkthrough next.
