Last updated: 12th August - Checked for new Anomalies.
The latest horror game to take Roblox by storm, Animal Hospital is another one of those pesky Anomaly-hunting games, where you need to greet customers, give them what they need, and, of course, make sure they aren't secretly a gremlin with spaghetti hands wearing something else's skin. Just an average customer service role, really, but those spaghetti-handed goblins can be mighty elusive, and we're here to help.
On this page, we've listed every Anomaly in Animal Hospital so you can survive the night, as well as providing key intel on how to spot the signs of each Anomaly and the method to combat them.
What's new in Animal Hospital's latest update?
The latest update for Animal Hospital added the Rain event, but no new Anomalies. We'll check for new Anomalies with the next update!

Animal Hospital Anomalies: The basics
So, how do Anomalies in Animal Hospital work in the first place? Every time you boot up a shift of Animal Hospital, you'll receive a set number of patients for the day. From that moment onwards, you'll have a randomised number of anomalies show up, encountering everything from roaming Skinwalkers and translucent men to a giant shadow that stalks you from afar and a glob of flesh and eyes that hangs from the ceiling.
I'd love to "speed it up a bit", pal, but I'm a little preoccupied with Mr No Eyes trying to exhume my soul at window 2. | Image credit: Eurogamer / Animal Anomaly
Although the game doesn't categorise these threats, I've opted to separate them into two distinct camps: Hazard Anomalies and Patient Anomalies. Below, you'll find a list for each of these categories, with Hazards counting as unavoidable Anomalies that show up throughout the hospital, forcing you to perform a specific action to escape, and Patients being spooky customers that you'll have to catch out and send packing before they can snack on your bones.
Need some tips for identifying Anomalies? Click here!An extra tip from Callum
Howdy, gang. Callum here. If you keep missing Anomalies, especially at the reception desk, there are a few simple routines you can start using to make sure you never miss any creepy gremlins crawling around your hospital.
First up, when checking a patient in, immediately stamp their details and take a photo of them. While the photo develops, go to the CCTV cameras, check them out there to make sure they aren't secretly sporting crinkly extended limbs or something, then go back and look at the photo. If it's clear, they're clear, so you can proceed.
Some other good tips. When the taser recharges, make sure to add it to your inventory so you don't need to bolt back to reception when a Skinwalker starts snacking on poor Whiskers McGraw on the other side of the building. The same deal applies to coffee. Keeping your inventory stocked will let you adapt more fluidly to situations as they arise and cut down the time it takes to backtrack to the reception desk.
All Anomalies
Here's every single Anomaly from both categories in a single list. You can skip down to their location on the page by clicking on whichever Anomaly tickles your interest:
Below, I've listed what I refer to as Hazard Anomalies. These are unavoidable threats and distractions that will show up around the Hospital while you work, attempting to kill you or your patients.
Flesh Ceiling
Appearance
A blob of flesh with eyeballs on the ceiling.
Anomaly Evidence
You'll hear groaning and see dripping saliva from the ceiling, not to mention the game will flash a handy "don't look up" on the screen. If you do, the flesh will stare at you and drain your Sanity meter.
Method of removing Anomaly
Avoid eye contact and treat the patient inside the room. The flesh will depart by the next shift. Alternatively, you can offer him eye drops to make him docile, so you can stare into his dreamy eyes all night long.
Hiders
Appearance
Translucent men who will blend into the tiles of a patient's room.
Anomaly Evidence
While in a patient's room, you may spot a pair of eyes and a moving body against the tiles on the wall. Staring at these eyes will cause the body to begin chasing you. Bro clearly nicked his entire strat off that one invisible dude from Exit 8, and he's fuming you saw through it.
Method of removing Anomaly
Run away and, upon reaching the lobby, he'll disappear in a puff of smoke. You can also zap him with the Taser or shoot him with the gun.
Skinwalker
Appearance
A monster with a large mouth that initially disguises itself as another patient.
Anomaly Evidence
Listen for heavy breathing and look out for an exact copy of a Patient you've already seen moving through the Hospital. That can be tricky to spot, so if you miss it, look out for the alert warning that comes up telling you a Skinwalker is attacking one of your patients, then run to find them before the time limit expires.
Method of removing Anomaly
Zap 'em with the taser to take them down quick, or you can hold E to forcefully dislodge them from their victim. If they attack you, spam E to punch that sucker's lights out.
Monster Under The Bed
Appearance
A dark shadow with bright red eyes and large black hands that hides underneath beds.
Anomaly Evidence
Upon entering a patient's room, you'll see a red line extending from the bottom of the bed and a pair of giant black hands beside it. If you look under the bed, you'll see red eyes staring back at you. Standing within the red line will hurt. A lot.
Method of removing Anomaly
Avoid the red line and treat the patient. The easiest method is to hop over the patient's bed. Who cares if you step into their open chest cavity? You have monsters to evade!
The Stalker
Appearance
A giant, shadowy bunny entity.
Anomaly Evidence
While exploring the hospital, you'll occasionally spot a towering shadow bunny that loves peeking around corners. Yeah, something tells me he's not just looking for a flu jab...
Method of removing Anomaly
Avoid making eye contact with the stalker if possible, but if you do, it's not a massive deal. You'll just lose 10 Sanity.
Demon cat jumpscare
Appearance
A demonic cat entity with glowing red eyes, only visible through the surveillance cameras.
Anomaly Evidence
When flicking through cams, you may spot a demonic cat standing at the end of one of the two hallways. The camera will zoom in on the cat, cueing a jumpscare.
Method of removing Anomaly
You can't really avoid this one outside of just not going on cams. Once it jumpscares you, you'll lose 20 Sanity.
Death Ritual
Appearance
A patient floating above their bed, surrounded by candles and red light.
Anomaly Evidence
Outside of hearing ominous sounds and red light emerging from the patient's room, you'll also get a handy emergency prompt telling you a patient is currently in a Death Ritual. When you arrive, the patient is floating above the bed, and all the nearby tech will have glitched out.
Method of removing Anomaly
Beat the timer and get to their room, then remove all the candles involved in the ritual. Be wary, though. Removing each candle will deplete your own Sanity. You can avoid Sanity loss by using eyedrops or coffee to extinguish the Candles. Use eyedrops. Coffee recovers Sanity, so the trade-off isn't really worth it.
The Head Knocker
Appearance
A patient with sunken, hollowed-out eyes.
Anomaly Evidence
At a random point, the player will return to the lobby to find a patient with no eyes bashing their head against the window on the right-hand side of the reception desk.
Method of removing Anomaly
Offer the patient some coffee, and they'll chill out, then leave. Alternatively, you can zap them with the Taser or shoot them with the Gun.
Tendril Surgery
Appearance
Purple tentacles sprouting from a patient's body.
Anomaly Evidence
While performing surgery, you'll occasionally see purple tendrils emerging from your patient on the operating table. You'll be tasked with performing this surgery immediately to ensure the patient survives.
Method of removing Anomaly
Proceed with the surgery as normal. When the mini-game is complete, the tendrils will be vanquished to where they came. BEGONE, FOUL BEAST OF THE FORGOTTEN SEA!
Slime
Appearance
Bright green slime on the floor.
Anomaly Evidence
On occasion, you'll spot a patient leaking bright green slime onto the floor as they walk by.
Method of removing Anomaly
Use a Fire Extinguisher to instantly clear Slime. Or, you can interact with each glob of slime to clean it up... very slowly...
Fire
Appearance
It's... uh, well... fire, you know? Orange and fiery...
Anomaly Evidence
Sometimes an empty patient room will catch on fire, engulfing the area.
Method of removing Anomaly
Grab the fire extinguisher on the right of the doors to the shop, then head back and extinguish the fire.
Fake surgery patient
Appearance
A regular patient.
Anomaly Evidence
When you're about to do surgery, your patient can pop up off the bed and reveal themselves to be a skinwalker, cornering you in the surgery room. They've played a blinder with that one, to be fair to them.
Method of removing Anomaly
Stun the patient with the taser.
Ghosts
Appearance
An invisible presence that can be sprayed with a fire extinguisher to reveal a floating, white, ghostly figure.
Anomaly Evidence
If a patient dies in your hospital, you won't just have the weight of your failure to save them dragging you down. Their ghostly remains will come back to physically drain your Sanity too. Ghosts are invisible at first and can be spotted by doors opening seemingly on their own or ominous sound effects playing nearby. When sprayed with a fire extinguisher, you'll then be able to expose the ghost’s physical form.
Method of removing Anomaly
Stun them with a taser or shoot them with the gun.
Toxic Shift event
Appearance
The hospital fills with a green mist, and chunks of slime appear around the building.
Anomaly Evidence
Usually on your twelfth shift, you'll get a message indicating that the hospital is changing, and green mist will fill the corridors as blotches of slime grow on the walls. Shortly after, a man in a hazmat suit will enter, ask you to sign something, then the shift will begin. It'll be a normal shift, except Slime will grow everywhere and you'll have to deal with Slimebert.
Method of removing Anomaly
Complete the shift to end the event.
Slimebert
Appearance
A giant roaming slime slug with a gaping mouth.
Anomaly Evidence
Upon beginning a Toxic Shift, Slimebert will spawn and come talk to you at the front desk. He'll then give up on checking in and roam around the hospital, trying to eat any character he can find, including you, the Animal Corporation officers in the hazmat suits and your patients. He'll never kill any NPCs, but he can knock them unconscious, forcing you to save them quickly.
Method of removing Anomaly
You can't kill Slimebert, but you can incapacitate him. Either spray him with the fire extinguisher, or hold some Cough Syrup in your hands when he attacks you. Both will melt him into a pile of sludge for a short amount of time.
All Patient Anomalies

Below, we've listed all the Anomalies that you'll spot while signing patients in at the reception desk:
Limb spasms
Appearance
A regular patient.
Anomaly Evidence
As the patient comes to the desk, they'll appear normal. However, if you watch them, you'll see their limbs spasm at random, with their neck craning backwards and their arm flailing at unnatural angles.
Method of removing Anomaly
Close the shutter.
Limb stretching
Appearance
A regular patient.
Anomaly Evidence
Will look like a normal patient, but the moment you look at them through the surveillance cameras, you'll clock that their limbs are stretched and often fully detached.
Method of removing Anomaly
Close the shutter.
Three eyes
Appearance
A patient with three glowing red eyes.
Anomaly Evidence
A patient will approach the desk with three glowing red eyes.
Method of removing Anomaly
Close the shutter.
Camera Watcher
Appearance
A regular patient.
Anomaly Evidence
A regular patient who, when viewed through the surveillance cameras, will make eye contact with the camera.
Method of removing Anomaly
Close the shutter.
Shadow patient
Appearance
A regular patient.
Anomaly Evidence
A regular patient who, when viewed through the surveillance cameras, will turn into a shadow.
Method of removing Anomaly
Close the shutter.
Poorly drawn patient
Appearance
A patient with hand-drawn, misproportioned eyes and a large mouth with crooked teeth.
Anomaly Evidence
The patient rocks up at the desk and... yeah, gonna go out on a limb and say that's not a real face. I appreciate the effort he put in, though.
Method of removing Anomaly
Close the shutter.
Big Teeth
Appearance
A patient with beady eyes and a big set of clenched teeth.
Anomaly Evidence
Again, this guy really isn't convincing anyone he's just a regular critter from the local woods...
Method of removing Anomaly
Close the shutter.
Hollow patient
Appearance
A patient with hollowed eyes and a blank expression.
Anomaly Evidence
Hollowed eyes are the textbook sign of "this guy's probably bad news." After all, if something hollowed out my eyes, I'd be moaning to everyone about it, and this guy's chill as hell.
Method of removing Anomaly
Close the shutter.
JPEG Eyes
Appearance
A regular patient.
Anomaly Evidence
A regular patient will approach the desk. Looking through the surveillance cams, you'll see a JPEG of a set of white eyes on a black background overlaid onto said patient, staring into the camera.
Method of removing Anomaly
Close the shutter.
Skinwalker At Reception
Appearance
A regular patient.
Anomaly Evidence
A regular patient will approach the desk, but if you look through the cams, you'll see it has a large open mouth with big teeth and is staring directly at the camera.
Method of removing Anomaly
Close the shutter.
Photo Evidence
Appearance
N/A
Anomaly Evidence
This covers a wide variety of Anomalies, but always check the polaroid you take when getting a patient's info. Occasionally, you'll see it has a horrifying visage of the patient. If the picture doesn't match 1:1, the patient is an Anomaly.
Method of removing Anomaly
Close the shutter.
Wrong Patient Photo
Appearance
N/A
Anomaly Evidence
This largely meshes with the photo evidence entry, but is slightly more subtle. Look at a patient's photo and really cross-check it against the patient that's arrived at the reception desk. Sometimes they'll have minor differences, such as different ears, eyes or mouths. If the picture isn't identical to the patient, you have an Anomaly on your hands.
Method of removing Anomaly
Close the shutter.
Staring patient
Appearance
N/A
Anomaly Evidence
Whether a regular patient or an anomalous one, you'll spot that your patient stares at you as you move around the office. Intense eye contact is definitely not a vibe... get rid of these weirdos.
Method of removing Anomaly
Close the shutter.
Stalker check-in
Appearance
A tall, dark, smiling figure (the same Stalker enemy you sometimes find peeping around corners).
Anomaly Evidence
On occasion, the Stalker will enter through the front doors and approach reception, craning its body to look through the window. It'll then offer you an Animal Corporation application form. Would love to accept this one, pal, but you gotta get the right documentation if I'm gonna apply maple syrup to your wounds.
Method of removing Anomaly
Once you take a picture of the Stalker, it'll vanish.
CCTV patient switch
Appearance
N/A
Anomaly Evidence
When checking the cameras, you'll sometimes notice that the patient at the desk on the monitor has different ears from the one at the counter. If so, you've got an anomaly on your hands.
Method of removing Anomaly
Close the shutter.
Tiny Anomaly
Appearance
N/A
Anomaly Evidence
A regular Anomalous patient, but tiny. So tiny they can't even see over the counter. I'm sure he wants to be scary, but he's very cute to be honest...
Method of removing Anomaly
Close the shutter.
And with that, it's time to go out there and protect your patients! Those spaghetti-hand-wielding gremlins are relentless, after all.
