Seconds after Geralt, Ciri, and Yennefer go through the portal to take Voleth Meir back to her home world, a pillar falls in the great hall, Jaskier sees it falling and pushes Vesemir out of the way, saving his life but is crushed by the pillar himself.
With Yennefer’s magic only just returned to her, she can only stabalize his injuries not heal them fully, the Witcher’s rally round to keep Jaskier comfortable and help him adjust to a very different life. Geralt battles with his guilt for abandoning Jaskier on the mountain and bringing him to Kaer Morhen leading to his injuries while trying to deal with his feelings for the Bard.
Whether or not Jaskier’s injuries are permanent or if they get healed is up to the writer.

Geralt had arranged to meet Jaskier in the Spring after he had spent Winter in Kaer Morhen only Jaskier does not arrive at their designated meeting point. Geralt heads for the nearest town thinking he’ll find Jaskier there but instead a messenger from Oxenfurt reaches him telling him that Jaskier fell dangerously ill over the winter.
Not knowing if Jaskier survived since the message is weeks old Geralt heads for Oxenfurt where he finds Jaskier alive but has been greatly weakened after weeks of illness and is also living in terrible conditions as he hadn’t been well enough to earn his keep.
Jaskier expects that Geralt will leave him to go on the path but Geralt decides to stay and get his Bard healthy again.
Meredid has fallen ill, and Emhyr finds ways to take care of him - maybe no one can notice, or maybe Emhyr doesn’t care if anyone notices.
