According to Will Luke:
Just found what I think might be a bug with the plugin under WP2.1.2. Specifically in the new fancy Ajax comments page (/wp-admin/edit-comments.php). When you approve comments that are pending moderation, no emails are sent out...either to the author or the recipient.
