fix(server): truncated Claude task output no longer starts with a replacement character - #28643
tailClaudeTaskOutput streams a Claude task's .output file as live command output. When the file shrinks, it rewinds the read offset to 0 but keeps the same streaming TextDecoder. If the previous read ended inside a multi-byte character, the decoder still holds those bytes, so they get joined to the first byte of the rewritten file and the new output starts with .
Expected: output after a truncation decodes on its own.
Change
Create a new decoder when the rewind happens. The skip-ahead path right below already resets the decoder for the same reason (its comment says "Reset partial UTF-8 bytes when skipping output"); the truncation path was the one place the offset jumped without it.
Scope and approval
No prior issue, under the very small, focused fix for an obvious bug route. The function already handles a shrinking file, and that handling has a defect: bytes from the old contents leak into the new contents. The fix is 3 lines in one function and changes nothing else about tailing, including the unbroken and skip-ahead paths.
Verification
Added one case to ClaudeTaskOutputTail.test.ts: write the first 4 bytes of café (ends inside the é), take the caf chunk, overwrite the file with ok\n, then assert the next chunk. It uses a real temp file with the live clock, like the other cases there.
cd apps/server && vp test run src/orchestration-v2/Adapters/ClaudeTaskOutputTail.test.ts
Before the fix: 1 failed, 4 passed. AssertionError: expected 'ok\n' to equal 'ok\n'
After the fix: 5 passed.
vp run check:changed passes (format, lint, knip, typecheck, tests beside the change).
Not checked: a real Claude session truncating its output file. I reproduced this at the tail function with a real file, and I do not know how often Claude Code truncates these files in practice, so the visible effect is one wrong character in live output, in an uncommon case. The final tool result is not affected.
Macroscope's review found this PR approvable — The change resets UTF-8 decoding state when a Claude task output file is truncated, preventing stale partial characters from affecting subsequent live output. Its effects are limited to text presentation, with existing file-reading behavior unchanged.

Walkthrough
The output tailer now resets its UTF-8 decoder when the file shrinks below its read offset. A live test verifies that the tailer emits rewritten content after truncation during an incomplete multibyte character.

Summary
Reset decoder state on truncation apps/server/src/orchestration-v2/Adapters/ClaudeTaskOutputTail.ts, apps/server/src/orchestration-v2/Adapters/ClaudeTaskOutputTail.test.ts
When the output file shrinks below the current read offset, the tailer resets the offset and decoder. A live test checks that rewritten content is received after truncation during a split character.

Explanation
The title names the main change: truncated Claude task output no longer starts with a replacement character. It also names the scope (server) and uses a conventional fix() prefix. The title matches the decoder reset on truncation in tailClaudeTaskOutput. It is 87 characters, which is long but still a single readable sentence.
Full details: Description check
Explanation
The description follows the template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section explains the stale TextDecoder after truncation, the expected behavior, and the reproduction path. The Change section explains the decoder reset and cites the existing skip-ahead reset as precedent. The Verification section gives the focused test command, the before and after results, and states what was not checked. The Scope and approval section invokes the very small obvious-bug route and gives a reason, but it does not link a triaged issue or maintainer approval. The template allows this route for a very small, focused fix, and the author explains why it qualifies. The description also ends with the model and harness, as the template asks.

Use it.effect and complete the stop deferred after the rewrite. That triggers the final read without waiting for the wall-clock poll. (raw.githubusercontent.com)

verified unchanged head 3a6b3e8d5cc953fea3c148b286fab6f4ecd9bc47 on linux, against github/main at 2b1021ce41. each revision ran separately with vp run dev and isolated state. i created local git projects and submitted real provider turns through the web composer using repository playwright and google chrome.
claude haiku 5.5 ran a foreground bash command that wrote an incomplete utf-8 character, paused, rewrote its own actual claude task output file to ok\n, then appended unicode output. this exercised real task_started frames, the tailer, server, and live client output.
main reproduced pr28643:cafok in live command output.
the pr showed pr28643:cafok, without the replacement character.
later append café 😀 streamed correctly. the final tool result remained ok, append café 😀, finish-ok.
ordinary streaming preserved é and 😀 split across separate writes without truncation.
real codex gpt-6.1-sol turns returned the expected unicode command output and exit code 0 on both revisions.
all five existing tailer tests passed on blacksmith after remote dependency installation. the pr's ci checks have no failures or pending runs.
electron shell, native ios/android, macos/windows, and relay/tunnel connections were not exercised on this linux vm. the truncation was deliberately caused by a real provider task; i did not establish when claude spontaneously truncates these files. the recording demonstrates output states, not performance.
