Not every care transition challenge is about coordination. Sometimes, it's about helping a patient feel ready to accept care. Mrs. Smith was reluctant to move forward with her care plan after being discharged. She declined services, postponed visits, and remained unsure about the next steps despite ongoing care needs. The resources were available, but the path forward was unclear. Holly, a WellSky transitional care coordinator who works as an extension of a value-based physician group to connect clinical needs with patient preferences, focused first on building trust. She listened to the Mrs. Smith's husband’s concerns, offered flexible options such as telehealth, and worked with the patient to identify a next step that felt manageable. That approach helped re-engage Mrs. Smith and her husband in care, and Holly was able to get an in-home clinical visit scheduled and support services in place. Even when services are available, progress depends on patient readiness.
