High-quality clinical documentation is a key element of driving value-based care success. Indiana Health Centers, Inc., part of the MHN Health Alliance ACO in the Medicare Shared Savings Program, meaningfully improved clinical documentation accuracy. A recent The New York Times Times investigation shed light on the soaring costs of certain skin substitute products in Medicare.
